As part of this mentorship program, mentees may have the opportunity to observe, discuss, or assist with responsibilities that involve confidential patient information.

The purpose of this HIPAA agreement is to ensure that all mentees understand their responsibility to protect patient privacy. Any patient information seen, heard, or discussed during the mentorship experience must remain confidential and should only be used for the purpose of learning or completing assigned program responsibilities.
